  2. Contact Number: Provide either the recipient’s room phone or the hospital’s main number (we recommend the hospital’s number as room numbers may change).

  4. Delivery Information: Select “Delivery” and enter the complete hospital name and address. If you know the room number, include it, but it’s not required.
